Required Credentials and Certifications

1. Prime CA Sexual Harassment Prevention Training

   • Valid for 2 years

2. Nursys License

   • Must be confirmed at Nursys

3. ACLS Certification

   • From American Heart Association or Red Cross
   • Required for onboarding

4. NIHSS Certification

   • Required for onboarding

5. Prime Basic Pharmacology Exam

   • Must score 80% or higher
   • Required for onboarding

6. Driver's License/State ID

   • Required for onboarding

7. Prime Drug Screening

   • Minimum 10-panel test
   • Required for onboarding

8. Prime Color-blind Test

   • Must be completed annually
   • Required for onboarding

9. COVID-19 Vaccine

   • Proof of 2 vaccines and booster required or declination completed
   • Required for onboarding

10. MMR Immunization

    • Proof of 2 vaccines or positive titer showing immunity
    • Required for onboarding

11. Hepatitis B Immunization

    • Required for onboarding

12. Influenza Vaccine

    • Seasonal flu vaccine or declination for 2024-2025

13. TB Test

    • Annual requirement

14. Physical/Health Statement
