Job Description

• Schedule: Monday to Friday, with hours between 0650-1520 (flexible shifts). Call required as needed, currently 1 day per week and 1 weekend per month. Holiday call hours will rotate with regular staff.
• Experience Required:

  • Minimum of 2 years of Open Heart experience.
  • Additional experience in robotic and general service lines, Cardiac, and Vascular preferred.
• Floating Requirement: May need to float onsite within skill set or to sites within 40 miles if needed.

Required Credentials

1. Ascension Specialty Exam: Must be completed prior to the first assignment and updated every 3 years. Minimum 80% passing score required.
2. BLS Certification: Must be AHA certified.
3. License/Certificate Verification: Current license verification required within 30 days prior to the start of the assignment.
4. Ascension Core Competencies: Must complete HIPAA, Joint Commission - NPSG, OSHA Topics training within a year of start and updated yearly.
5. MMR Vaccination: Documented proof of two MMR vaccines or positive IGG titer required.
6. Fit Test: Required for all travelers, preferred to be completed prior to start.
7. Hepatitis B Vaccination: Documentation of vaccination series completion or declination required.
8. Influenza Vaccine: Required during flu season (October 1 - March 31). Exemptions accepted with mask requirement.
9. Health Statement: Statement of good health or completed physical exam required prior to the first assignment.
10. Driver's License/State ID: Must not be expired and match the address on file.
11. 2 Step PPD or IGRA: Required prior to start.
12. Varicella Vaccination: Documented proof required.
13. Tdap Vaccination: Must be valid for 10 years from the administered date.
14. Ascension Profile: Resume or profile submission required.
15. Professional References: At least two professional references required.
16. Criminal Background Check: Must be completed within 30 days of the first assignment.
17. Education Verification: Verification of education relevant to clinical work required within 90 days of start.
18. 12 Panel Drug Screen: Must be completed within 30 days prior to the start of the assignment.
19. Ascension Badge Photo: Forward-facing headshot required.
20. Confirmation of Assignment: Must be provided upon initial assignment and updated upon each extension.

Additional Information

• Exit Survey: To be completed within 72 hours at the completion of the assignment.
• Employment Verification: Third-party verification for the past seven years required.
