Certifications

• BLS (Basic Life Support): Required
• ACLS (Advanced Cardiovascular Life Support): Required

Additional Credentials

• License/Certificate Verification: Must be dated within 30 days of start.
• Fit Test: Required.
• Trinity Code of Conduct Form: Must have page 27 signed.
• Trinity I9: Standard I9 completed by the agency.
• Confidentiality Agreement: Signed form on file.
• Integrity and Audit Services Form: Required.
• Respirator Evaluation: Physician signature required.
• Specialty Specific Competency Testing: Must be completed within a year.
• Skills Checklist: Required for clinical contractors.
• 2 Step PPD OR IGRA (Quantiferon): Required.
• Trinity MA CORI: Must be completed within 30 days of start.
• Trinity OFAC: Must be completed within 30 days of start.

Vaccination Requirements

• COVID-19 Vaccine: Proof of vaccination, declination, or exemption required.
• MMR (Measles, Mumps, Rubella): Proof of vaccination required.
• Hepatitis B: Proof of vaccination required.
• Influenza Vaccine: Proof of vaccination required during flu season.
• Varicella: Proof of vaccination required.
• Tdap: Current vaccination required.
• Physical/Health Statement: Documentation from an exam within the last year required.
• 9 Panel Drug Screen: Must be completed within 30 days of start.

Background Checks

• Criminal Background Check: Must be dated within 30 days of start.
• Education Verification: Attach completed report of highest level of education.
• EPLS and GSA/OIG Checks: Must be dated within 30 days of start.
• NSO Check: Must be dated within 30 days of start.

Documentation
