  • Job Title: Care Transitions Case Manager
  • Department/Subsidiary: Care Management (a sub-department of Acute Population Care Management)
  • Job Code: Y0423
  • Effective Date: March 2021
  • The Care Transitions Case Manager collaborates with physicians, social services, nurses, community agencies and other members of the care team to define care options and resources, plan cost-effective quality care, and achieve optimal quality outcomes.
    Job requirements
  • BLS certification required for all direct patient care personnel.
  • License/Certificate Verification must be dated within 30 days of start.
  • NIHSS certification required for this role.
  • All engaged contractors must complete the required health and professional screenings.
  • Must attach scanned proof of BLS certification (with expiration date) to candidate's record.
    Schedule information
  • 12 weeks contract
  • 40 hrs/wk
  • Shift: 7A-3:30P (1/2 hour lunch)
    Unit specific information
  • Coordinates with social workers to ensure a warm handoff with caregivers at the next site of care.
    Additional Details
  • Must include an image of the front and back of the BLS certification card.
  • Federal, Nationwide, County Criminal check from last 7 years required.
  • Iowa's Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required for all engaged contractors.

My review isn’t about the recruiter at all. The position for Rochester, NY psychiatry, I don’t recommend this position. It turns into 60 hours of straight pay, they take advantage of their travelers, no weekends off, 5-6 days a week depending on when they want to put you on call. They schedule you 5 days a week to tell you it’s only 4 however your agency will pay you for the 60 hours, which isn’t what anyone signs up for. They don’t tell you that you have to work 60 hours of on call until you get there. You will work 5-6 days a week here.
