- Primary Source Verification of professional license and/or certification, as required for the position ran within 30 days of start updated upon expiration. Attestations not allowed. Needs to be rerun every 2 years with RN license renewal & upon expiration. Temporary Licenses are NOT accepted – must have a valid permanent license.
- Agency to provide completed I-9 or attestation that completed I-9 is on file and candidate is eligible for work.
- Ran within 6 months of Start. Search includes: SSN Trace and County searches for the last 7 years of locations lived/worked/in school. National Criminal Sweep to also include NSO. Only need to complete a new BGC if break in assignment 6m+.
- Live in Ohio for 5 years: Ohio Fingerprints (BCI&I check) AND FCRA-Compliant National Database Criminal background check. In lieu of National Crim. BGC, the FBI fingerprint check (National coverage, must be FCRA compliant) acceptable.
- Highest level of education verified. Attestations not allowed. If education was obtained internationally, equivalency document must be provided from an accredited organization.
- Must provide verification of employment within 5 years. Must be completed through third-party verification. Agency does not need to confirm employment that occurred at their agency. Attestations not allowed.
- Competency Exams for the relevant department. Must score at least 80%. To be complete within 1 year prior to start. Copies of exam required. Attestations not allowed.
- 80% passing score required, Completed within 1 year prior to start. HIPAA, Joint Commission-NPSG, OSHA Topics. CORE 1, 2 and 3 must be uploaded. Attestations not allowed.
- Physical Exam completed within one year of initial start date. Needs to be a Pre-Employment exam statement signed and must state that traveler is able to work without restrictions. Attestations not allowed.
- Needs to be filled out and uploaded by agency upon acceptance. Agency representative signature required.
- Safety and Compliance Training modules emailed to agency POC by UHS after acceptance. Candidates need to complete module prior to clearance. Once complete, please upload the certificate of completion.
- Clinical Only: Skill specific required to be complete within 1 year prior to start. Only needs to be done once upon initial hire.
- Proof of vaccine series or completion and approval of client medical or religious exemption form. Preferred, not required.
- Positive MMR titers ONLY from within the last 15 years.
- Completed within 1 year of start updated annually. Cannot be done on site.
- Will accept one of the following: (a) Proof of full Hepatitis B series completed (b) At least two vaccines from the series completed, or (c) signed declination. Positive titers also accepted within 15 years.
- Proof of vaccine from Oct-March or exemptions must be submitted for approval.
- If positive, CXR within 5 years and current Questionnaire
- 2-Step TB test with negative reads within the last 6 months.
- Positive Varicella titers ONLY from within the last 15 years.
- Proof of vaccination within past 10 years, declinations not accepted.
- If 2 step PPD not done within past year.
- For Ohio residents to meet fingerprint requirements, please upload FCRA-Compliant National Database Criminal background check.

My review isn’t about the recruiter at all. The position for Rochester, NY psychiatry, I don’t recommend this position. It turns into 60 hours of straight pay, they take advantage of their travelers, no weekends off, 5-6 days a week depending on when they want to put you on call. They schedule you 5 days a week to tell you it’s only 4 however your agency will pay you for the 60 hours, which isn’t what anyone signs up for. They don’t tell you that you have to work 60 hours of on call until you get there. You will work 5-6 days a week here.
