Required Credentials and Certifications

1. BLS Certification

   • American Heart Association certification required for all direct patient care personnel.
   • Attach scanned copy of certification (with expiration date) to candidate's record.

2. License/Certificate Verification

   • Must be dated within 30 days of start.
   • Attach scanned online verification page with a machine-generated date indication.

3. NRP Certification

   • Required for RNs in Birth Center.
   • Attach scanned copy of certification (with expiration date).

4. AWONN/Fetal Heart Monitoring Certification

   • Required for onboarding.

5. Trinity 9 Panel Drug Screen

   • Must be completed within 30 days of start date.
   • Must include specific substances (NO THC).

6. Trinity Code of Conduct Form

   • Must have page 27 signed form on file.

7. Trinity I9

   • Standard I9 completed by the agency.

8. Trinity Confidentiality Agreement

   • Must have page 5 signed form on file.

9. Trinity MVR (Motor Vehicle Report)

   • Must be dated within 30 days of start.

10. Trinity Skills Checklist

    • Required for clinical contractors.

11. 2 Step PPD OR IGRA (Quantiferon/Tspot)

    • Must be completed within 30 days of start.

12. COVID-19 Vaccine

    • Proof of vaccine, declination, or exemption required.

13. MMR Vaccination

    • Proof of vaccination required.

14. Hepatitis B Vaccination

    • Must have 1st vaccination prior to start.

15. Influenza Vaccine

    • Proof of vaccine, declination, or exemption required.

16. Varicella Vaccination

    • Proof of vaccination required.

17. Tdap Vaccination

    • Current, non-expired TDAP vaccine required.

18. Physical/Health Statement

    • Documentation from an exam within the last year required.

19. Trinity Profile

    • Resume, Skills Checklist within a year, and 2 Professional References required.

20. Trinity Conflict Of Interest Disclosure Form

    • Must have page 1 signed form on file.

21. Trinity Criminal Background Check

    • Must be dated within 30 days of start.

22. Trinity Education Verification

    • Attach completed report of highest level of academic education.

23. Trinity EPLS

    • Attach scanned EPLS report dated within past 30 days.

24. Trinity GSA/OIG

    • Attach scanned OIG result print-out dated within past 30 days.

25. Trinity NSO

    • Must be dated within 30 days of start.

26. Trinity System Access Form

    • Must be completed and uploaded as soon as possible.
