Required Credentials and Qualifications

1. License/Certificate Verification

   • Primary Source Verification of professional license and/or certification required within 30 days of start and updated upon expiration. Temporary licenses are not accepted.

2. Fit Test

   • Must be completed within 1 year of start and updated annually.

3. ARRT Certification

   • Required for onboarding.

4. UHS Skills Checklist

   • Skill-specific checklist to be completed within 1 year prior to start.

5. UHS Competency Exam

   • Must score at least 80%. To be completed within 1 year prior to start.

6. UHS Core Competencies

   • 80% passing score required, completed within 1 year prior to start.

7. COVID-19 Vaccine

   • Proof of vaccine series or approved medical/religious exemption preferred.

8. MMR

   • Positive titers required from within the last 15 years.

9. Hepatitis B

   • Proof of vaccination series or signed declination required.

10. Influenza Vaccine

    • Proof of vaccination from October to March or approved exemption.

11. 2-Step PPD or IGRA (Quantiferon/Tspot)

    • Negative results required within the last 6 months.

12. Varicella

    • Positive titers required from within the last 15 years.

13. Tdap

    • Proof of vaccination within the past 10 years.

14. UHS Submission Packet

    • Resume, education, work history, and 2 professional references required.

15. UHS I-9 Attestation

    • Completed I-9 or attestation required.

16. UHS Criminal Background Check Results

    • Must be completed within 6 months prior to start.

17. UHS Education Verification

    • Verification of highest level of education required.

18. UHS Employment Verification

    • Verification of employment within the last 5 years required.

19. UHS OIG Search

    • To be completed within 6 months prior to start.

20. UHS Drug Screen

    • Must be completed within 60 days of start.

21. Physical/Health Statement

    • Pre-employment exam statement required, stating the traveler is able to work without restrictions.

22. UHS Module Completion Certificate

    • Safety and Compliance Training modules to be completed prior to clearance.

23. UHS National Database Criminal Background Check

    • For Ohio residents to meet fingerprint requirements.

• Background Check: Must include SSN Trace, County searches for the last 7 years, and National Criminal Sweep.
• Photo ID: Non-expired photo ID required.
• Headshot: Shoulders up, plain background, uploaded as JPEG only.
