Description
American Medical Staffing (AMS) puts you first, and we are currently seeking a Home Health LPN for a position in Concord, MA. Monday-Friday days.
Home Health LPN Job Details:- Pay Rate: $1,700-$1,900 gross per week
- Schedule: 8:00AM-5:00PM
- Available Shift: Monday-Friday, 40 hours per week
- Start Date: 1/6/2025
- Located In: Concord, MA
- Requirements: Qualified applicants MUST have at least 2 years of experience in home health, have a valid LPN license and be willing to obtain MA licensure.
As a Home Health LPN, you'll have the opportunity to provide comprehensive care to patients in the comfort of their own homes, fostering independence and promoting optimal health outcomes. As their in-home clinician, you will conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with their interdisciplinary team.
In the Home Health setting, you will be a vital link between a patient and their family to resources across the care spectrum. This mobile role offers independence and differentiation in your day-to-day work, while also keeping you connected to the coordination of care with other healthcare providers like physicians, therapists, and social workers.
