American Medical Staffing (AMS) puts you first, and we are currently seeking an Physical Therapist (PT) for a contract Home Health position in Worcester, MA.
Home Health PT Job Details:- Pay Rate: $2,245-$2,445 weekly gross
- Schedule: 8a – 5p
- Territory: Shrewsbury, Worcester, Millbury, Sutton, Douglas
- Available Shift: 8 hour days, day shift, Monday-Friday
- Start Date: 11/11/24
- Located In: Worcester, MA
- Requirements: Qualified applicants MUST have at least 1 years of experience in a Medicare-certified home health agency and be comfortable with OASIS (Outcome and Assessment Information Set), have a valid PT license and be willing to obtain MA licensure.
As a Home Health Physical Therapist (PT), you’ll have the opportunity to provide comprehensive therapy care to patients in the comfort of their own homes, fostering independence and promoting optimal health outcomes. This one-on-one healthcare approach allows you to connect and collaborate with individuals on their care, while acting as an advocate for their needs and quality of life. Responsibilities include:
- Conducting comprehensive assessments of patients' health status and developing individualized care plans in collaboration with the interdisciplinary team
- Providing therapy care to patients in their homes
- Performing OASIS assessments and documentation accurately and timely to ensure compliance with regulatory requirements
- Educating patients and their families on disease management, medication administration, and other aspects of health to promote self-care and independence
- Monitoring patients' progress and response to treatment, adjusting care plans as needed to achieve desired outcomes
- Providing emotional support and counseling to patients and families coping with chronic illness or end-of-life care
