Epic Travel Staffing is hiring a Travel - PT Home Health
Shift: Days, 8a-4:30p, 8x5, 40 hrs per week
Length: 13 weeks
1 Weekend Shift Per Contract
Requirements:

• CA license
• 1 year of Skilled Home Health experience
• BLS
• Oasis is preferred, but not required

Other Details:

• Traveler is expected to have their own transportation
• First-Timers Accepted: No
• Charting Epic
• COVID-19 Vaccine (Facility Guideline):Required + Booster - Medical/Religious Exemptions and Declinations Allowed
• Flu Vaccine (Facility Guideline):Unknown
• Locals Accepted: Yes; Considered "local" within 50 mile radius, will accept locals 

Home Health PT - Sonoma CA and surrounding areas- Start Date: ASAP- Shift: 8hr Days (*This facility cannot accommodate 4x10s or 3x12s.*)- Ratios: 1:1 (*May visit up to 6 patients per day; 30 productivity points standard)- Years of Experience REQ: 1 YR of Skilled Home Health Experience- First-Timers Accepted: No- Weekend REQ: 1 Weekend Shift Per Contract- Certs REQ: BCLS- Is On-Call REQ?: No- Will this traveler need to float between like units?: No- Open to accommodating block schedule?: No- Locals Accepted: Yes; Considered "local" within 50 mile radius, will accept locals - Guaranteed Hours: Guaranteed Work Week (per contract)! Client agrees to pay all weekly hours; travelers will not be called off shifts
Special Requirements- Traveler is expected to have their own transportation.- System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of 50+ miles.- Oasis is preferred, but not required; please note if candidate does not have Oasis experience
