Public Health Nurse with clinical duties
The Public Health Nurse will play a vital role in improving community health by managing patient care for our client on-site, in collaboration with the Clinical Team. This position combines clinical responsibilities with public health outreach to educate patients, reduce healthcare costs, and enhance patient and employee satisfaction.
Key Responsibilities:
Patient Care Management:
Conduct ongoing assessments of patient health risks and care gaps; proactively reach out to high-risk patients to facilitate preventive care.
Provide primary and urgent care services on-site within the RN scope of practice.
Clinical Duties:
Perform blood draws and conduct onsite rapid testing for a variety of conditions.
Ensure thorough documentation of patient information, visit histories, and follow-up actions in the EHR.
Proficient with technology and highly adaptive in changing environments.
Patient Education and Support:
Educate patients about disease prevention, chronic conditions, and injury prevention methods to promote wellness and self-management.
Create nursing care plans in collaboration with multidisciplinary care teams to achieve optimal patient outcomes.
Work closely with our client to share insights, improve care coordination, and enhance patient experiences
Collaboration and Care Coordination:
Serve as a liaison between patients, employers, and healthcare providers, facilitating necessary services and appointments.
Work closely with the Team and other health professionals to enhance care coordination and identify high-risk patients.
Community Health Initiatives:
Support corporate wellness objectives and strategic initiatives, promoting health literacy within the community.
Conduct follow-ups with patients post-injury or illness to ensure continuity of care.
Qualifications:
Education & Experience:
Bachelor’s degree in Nursing required.
Minimum of 3 years of clinical nursing/ambulatory care experience; experience with employer groups is preferred.
1+ year of experience in family practice, urgent care, pediatrics, or similar fields.
Licensure:
Registered Nurse (RN) licensed in the State of Minnesota.
Current BLS certification
Skills:
Strong communication and customer service skills.
Proficient in critical thinking and problem-solving in dynamic healthcare environments.
Comfortable using technology and EHR systems.
Preferred:
3+ years of clinical nursing experience.
Knowledge of health counseling and case management principles.
Job Type:Full-time position available. Normal hours are weekdays from 8AM to 5PM. This position is in-person, on-site in Delano, MN.
