Job Summary and QualificationsIn collaboration with the medical staff, patients, patient’s family and other members of the health care team, the Orthopedic Program Coordinator is responsible for utilizing a holistic and interdisciplinary approach to coordinate the care of all orthopedic patients. In addition, the Coordinator may also be responsible for assessing the needs of patients in collaboration with Nursing and Therapy Services, planning interventions and evaluating outcomes of care. The Orthopedic Program Coordinator may also provide direct care within their scope and practice and delegate/collaborate care with other disciplines and non-licensed staff as it relates to the outcome and care of the orthopedic patient. The Orthopedic Program Coordinator is responsible for providing care consistent with Medical City Las Colinas policies, procedures and standards of practice.
What you will do in this role:
- Monitors the provision of orthopedic patient care services from admission, stabilization, and through discharge
- Oversight of the Orthopedic Quality Assurance and Process Improvement (QAPI) initiatives
- Collaborate with Orthopedic Medical Director & Nursing Directors in areas to include, but not limited to: developing and/or revising policies, procedures and guidelines, and the QAPI Program
- Ensures ongoing family centered care practices
- Responsible for daily management of the QAPI program including data analysis, identification of trends and issues, medical record extrapolation, and development and implementation of action plans to address identified issues.
- Responsible for the daily management of Orthopedic care throughout the facility ensuring care is consistent with established practice standards and research based policy/procedures.
- Responsible for ensuring loop closure, including development and implementation of action plans, is obtained on identified PI issues and trends by collaborating with clinical leaders of departments caring for orthopedic patients.
- Develop patient/family education and programmatic outreach programs
- Provide age appropriate care to all patient populations for which program coordinator is responsible and trained for and demonstrates communication skills appropriate for age
- Practices autonomously, consistent with practice standards and research based policy/procedures
- Exercises independent judgment within the context of interdisciplinary patient care
- Provides care/treatment/services within the Synergy Model
- Maintains positive interpersonal relationships and self conduct to contribute positively to the delivery of quality care and services, and to orderly operation of the department and the facility
- Complete gap analysis for programmatic development for the advancement of Orthopedic quality care to include but not limited to: TJC Disease Site Certification, PRISM award recognition, support Unit of Distinction activities
- Cultural ambassador who participates in peer interviews, mentors other staff and peers, precepts employees and facilitates recognition and team building activities to support engagement and retention activities.
- Supports patient experience initiatives and assists with validations and coaching, patient rounding and discharge phone calls.
- Registered Nurse, or Licensed Physical Therapist Required
- Bachelors Degree, or Doctorate Degree, or Masters Degree
- Minimum of 2 years clinical experience in Orthopedics Required
- Expertise in clinical assessment for orthopedic patients.
- Ability to work independently and function as a part of an interdisciplinary team to continuously improve the process to enhance patient outcomes
- Effective communication and problem solving skills
- Knowledge base in adult learning theory. Able to apply knowledge to education offerings
- Participation in committees related to orthopedic care
