Job Summary and Qualifications

As an Admission/Discharge Registered Nurse, your primary responsibility is to ensure an effective discharge process for patients by assessing for needs and understanding of their care after they are discharged. You will also collaborate with inpatient directors to positively affect the patient’s overall experience. In addition to these responsibilities, you will:

• Assist with throughput by facilitating timely admissions and discharges
• ​Assist with and perform elements of patient admission to include, but not limited to the following upon patient arrival:
• • Admission Arrival
  • First Point of Contact
  • ​Admission History
  • ​Medication Reconciliation
  • ​Skin Assessment
• ​Serve as the liaison between the patient and care team to assist with coordinating discharge care.
• ​Initiate and document patient teaching, admission education, and discharge planning in collaboration with multidisciplinary team members
• ​Support care experience by assisting with discharge phone calls on inpatients; and provide relevant updates from phone calls as needed to Directors, ACNO, and CNO
• ​Communicate effectively with peers, supervisors, ancillary department, and other customers
• ​Perform other duties as assigned by immediate supervisor

What qualifications you will need:

• Nursing graduate with RN Diploma or Associate of Science in Nursing required​, BSN preferred
• Minimum of 2 years acute care nursing preferred
• BLS required
• Licensed a Registered Nurse in the Commonwealth of Virginia or has a Compact license
