Description
BJC HealthCare System is hiring a Full-Time Float MSW Social Worker - Inpatient Transplant for Barnes-Jewish Hospital (MCC) - Case Coordination
Position Details:
- Day Shift Hours: 8am-4:30pm (Mon-Fri)
- Role works with different organ groups to complete psychosocial assessments and follow up with post-transplant patients
- High organization skills, self -motivation, great team environment, fast pace
- Participate in IDR rounds with interdisciplinary team and provide updates on patient needs and discharge plans
- Complete psychosocial assessments and assist in resource connection, provide emotional support check in conversations post-transplant
- Daily Discharge Planning & Coordinate Referrals (skilled nursing facility, inpatient rehab, long term acute care)
Requirements:
- Master's Degree of Social Work (MSW) – required

Overview
Barnes-Jewish Hospital at Washington University Medical Center is the largest hospital in Missouri and is ranked as one of the nation's top hospitals by U.S. News & World Report. Barnes-Jewish Hospital's staff is composed of full-time academic faculty and community physicians of Washington University School of Medicine, supported by a house staff of residents, interns, fellows and other medical professionals. Recognizing its excellence in nursing care, Barnes-Jewish Hospital was the first adult hospital in Missouri to be certified as a Magnet Hospital by the American Nurses Credentialing Center.
The CM Services reports through the division of Patient Care Services lead by the Chief Nursing Executive, Vice President of Patient Care Services. The CM Service is comprised of three departments: Case Management, Utilization Management, and Social Work. Through the Triad Model of Care Delivery, the objectives of the CM Services are for all patients to have access to healthcare through the BJH continuum and appropriate, safe discharge plans. The SW department is open 24 hours/day while the CM department is open 20 hours/day. Both departments provide coverage throughout the hospital including the Emergency Department, Outpatient Clinics, Registration sites, and Health Information Management. The Service employs registered nurses, social workers, a license practical nurse, a customer service representative, and administrative assistants.

Role Purpose
Under direction of a physician, assists patients and their families in their residences or outpatient/inpatient settings to adapt and plan an interdisciplinary approach to treating the patient-family system.Employees working at an Illinois facility are required to have an Illinois state social work license or be able to take and pass the Illinois state social work licensing exam within 90 days of the first day of employment.
