Job Summary and QualificationsWe are seeking a RN Coordinator, Hematology who functions as a member of the multidisciplinary team and oversees and directs the care of the blood cancer patient and post-transplant patients requiring ongoing maintenance therapy. They function as an advocate and educator for patients from point of entry, diagnosis and treatment, through survivorship. Primary function is to establish a relationship with patients, family/caregivers, and physicians, and facilitate effective communication. They are responsible for coordinating the plan of care including appointments, patient and family/caregiver education, provision and/or enablement of support services, and follow-up. Serves as a liaison between the patient and family, primary care physician, internal and external care providers, specialists, referring providers, supportive care members e.g. social workers, hospital staff, and the wider healthcare community. Assumes responsibility and accountability for the management of resources to achieve efficient, high quality outcomes (e.g. Oncology Alert Program) for each blood cancer patient. What You Will Do In Your Role:
- Assess patients’ medical, social, psychosocial, and other care needs
- Provides appropriate teaching and education to patients and families/caregivers. The goal of this education is to ensure the patient feels empowered to manage his or her own health.
- Streamline appointments and paperwork by helping patients with scheduling appointments and preparation.
- Initiate referrals to hospital and community resources to connect patients with resources and support systems.
- Conduct follow-up conversations, as needed, with patients and communicate concerns, changes, or social needs in patient health to appropriate physician or other care providers.
- Attend patient care planning conference and other meetings, as necessary.
What qualifications you will need:
- Bachelor of Science in Nursing (BSN)
- Minimum of 3 years working in oncology setting.
Preferred:
- Master of Science in Nursing (MSN)
Employee completes initial and ongoing training and competencies as defined by service line, facility and department/unit which are specific to the needs of the patient population served (if applicable).
Minimum License, Certificates, or Registrations Required:
- RN License to practice as a professional registered nursing in the state of Texas or Active Multi-State RN Compact License, or GN Permit for RN’s being hired into RN I position.
- (Employees with RN Compact License are required to obtain Texas RN License within 90 days of hire date.)
- BLS Certification required
- Preferred: OCN
