Summary:

This Social Worker will provide behavioral health triage and perform intake assessment by phone and on-site in our Neighborhood Clinics to determine the appropriate level of care and treatment for established patients. The social worker will assess patients and accompanying family systems for needed assistance with resource procurement that aids the family's efforts in caring for their child's behavioral health needs. The social worker provides ongoing assessment, resource navigation, case management, limited supportive counseling, and/or crisis intervention as appropriate for NHC patients referred to the behavioral health team. The social worker collaborates with the behavioral health therapists to support behavioral services within the NHCs.

Note: Hybrid/Remote position covering several clinic locations
 

Education, Licensure & Required Skills:

• Master's degree in social work

• Three years of social work experience

• LMSW or LCSW Licensure with the state of Texas

• Experience working in a healthcare setting and/or providing behavioral health services preferred

• Bilingual English and Spanish preferred 

• Requires effective interpersonal skills including effective relationship-building with children and families.

• Ability to collaborate effectively as part of a team of professionals.

• Critical thinking, problem-solving, and self-motivated initiative

• Ability to plan and organize effectively

• Proficient in the use of computer applications
