EDUCATION:
- Master's of Social Work (MSW) from an accredited program required.
LICENSES, CERTIFICATIONS, and/or REGISTRATIONS (if applicable):
- Require current Massachusetts Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Social Worker (LCSW), or must obtain MA Licensed Clinical Social Worker (LCSW) within 4 months of hire date.
EXPERIENCE:
- Minimum 1-2 years clinical experience preferred.
- Experience with EPIC a plus.
- Experience with Veteran and military family population and knowledge of military culture a plus.
- Training in Trauma focused therapy modalities such as Prolonged Exposure and/or Cognitive Processing Therapy is preferred but not required. SKILLS/ABILITIES/COMPETENCIES REQUIRED:
- Exceptional organizational skills and ability to prioritize effectively and complete tasks; Able to juggle daily tasks with needed crisis intervention

Home Base seeks a dynamic and experienced Licensed Clinical Social Worker Licensed Independent Clinical Social Worker (LCSW / LICSW) to provide and oversee the provision of case management and clinical services for our patients. The LCSW / LICSW will be dedicated to supporting Home Base services, working primarily in the two-week Intensive Clinical Program (ICP). - Working under the supervision of the ICP Social Work Team Lead, the Director of Social Work, and the Director of the ICP,the Home Base LCSW / LICSW is responsible for coordinating care for Veterans and military-connected family members seeking care at the Home Base ICP with the appropriate internal or external clinicians in a multi-disciplinary team, coordinated care model.
- Serve as a central contact point for accepting and triaging referrals to the Intensive Clinical Program from internal clinicians, outside clinicians and self-referred patients.
- Serve as primary case manager for patients in ICP admissions process in collaboration with Admissions Coordinator, including, but not limited to, the following responsibilities: conduct pre-screening to assess treatment and evaluation needs for all patients referred; coordinate signing of Release of Information forms, acquire relevant medical records, and arrange for completion of toxicology screening; conduct family member pre-screen; serve as point of contact for ongoing questions from the patient as they arise.
- Track patient’s status in admissions process using appropriate documentation.
- Collaborate with patient, clinical team, and Resource Specialist to identify clinical and psychosocial needs in patient’s home community and identify appropriate resources.
- Provide direct clinical services to Veterans and/or family members, which may include groups such as Resilient Warrior, DBT, In Vivo, or individual evidence-based trauma focused treatments such as Prolonged Exposure Therapy or Cognitive Processing Therapy.
- Provide psychoeducation regarding PTSD, TBI, and related conditions to Veteran patients and family members as clinically appropriate within the context of the Intake and triage duties.
- Consult with team to facilitate mandated assessments when abuse is suspected (child, disabled adult, elder), and safety assessment when violence is reported. Collaborates with MGH resources (HAVEN and Child Protection Team)
- Responsible for crisis intervention and management
- Documents timely and relevant information in patient electronic medical record and case management database.
Maintain expected productivity expectations.
- Tracks and reports out on numbers of patients in admissions process and their status as required by funding sources.
- Meet with community and professional resources as requested by Home Base leadership to both represent Home Base and keep up to date on available community resources.
- Provide coverage for social work responsibilities in Outpatient Clinic as needed.
- Provide clinical supervision to LC-level and/or MSW students as needed.
- Additional responsibilities as assigned.
