FULL TIME/ 40 HOURS/ CHARLESTOWN, MA / CLINICAL SOCIAL WORKER LICSW - HYBRID work schedule including onsite and virtual/remote. - Seeking LICSW (May consider someone in process for independent license). - Excellent benefits: generous Paid Time Off; 403B match; cash balance pension; tuition reimbursement of $5,250/year, Continuing Education; Medical and Dental; Short-term disability; MBTA pass subsidy; and much more. - Full time job, but may consider less than full time or part time hours for qualified candidates. JOB HIGHLIGHTS - Provides clinical services with adult patients, couples, and/or families as part of a behavioral health team providing integrated care in a community health center. - Provides state of the Art Therapy dedicated to enhancing the emotional, social, and physical well-being of patients by employing patient centered, patient directed approach to care. - Conducts psychosocial assessments, collaborates to build treatment plans, and provides indicated therapy, utilizing evidence based and culturally sensitive models of care. - Collaborates with colleagues in Internal Medicine and other health center professionals in developing and implementing care plans. - Provides social work consultation to other disciplines within the Health Center and community. - Facilitates clinical education of social work students. - Participates on Departmental, Health Center, and Hospital committees. - Considers the appropriate and efficient use of Health Center resources and supports implementation of innovations in care delivery as appropriate. Clinical Practice - Provides psychosocial assessments of patients and families. Formulates diagnosis, disposition, and treatment plans. Employs clinical services such as individual, couples, family, and group therapy; provides crisis intervention; participates in case management and discharge planning. - Provides treatment that is evidence based, incorporating updated clinical care models. Develops effectivetreatment plans and modifies based on patient progress/outcomes. - Provides care that is culturally/racially sensitive, and trauma informed. Advocates on behalf of patients and/or their families to gain access to services and resources. - Presents and/or discusses clinical work in formal and informal settings and seminars. Seeks clinical consultation as needed. - Provides risk management. Facilitates the appropriate and efficient use of Health Center and Hospital resources. - Takes Urgent response duty as assigned; performs other duties as assigned. May participate in research projects. - May assist with or supervise undergraduate and graduate social work students and staff. - May include treating patients with Substance Use Disorders. Collaboration - Collaborates and consults with patient, families, professionals, and providers inside and outside the Health Center to develop and implement outpatient care. Collaborates with community-based resources as needed. - Participates as a key member of integrated medical and behavioral health care teams, participating in treatment planning and providing outpatient therapy. - Refers to and collaborates with interdepartmental specialists and programs, outside agencies, facilities, payors, and other relevant community resources. Organizational/Administrative Skills - Takes responsibility for own administrative duties, including timely and appropriate documentation in patient medical records. Effectively utilizes electronic medical record system to maintain documentation and communicate with other team members. - Monitors and manages productivity independently to established standards. - Attends and participates in team meetings, unit meetings, division meetings, and staff meetings. Professional Conduct - Adheres to the profession’s clinical standards and code of ethics. Adheres to the Department, Health Center and Hospital standards, policies and procedures. - Maintains the standard and educational requirements specific to the position and professional licensure. - Promotes Departmental, Health Center and Hospital public relations. - Professional Development - Participates in supervision as appropriate to his/her level of professional development. May assist with or supervise undergraduate and graduate social work students and staff. - Is responsible for own professional development. Identifies own learning needs and goals. Attends pertinent continuing education and professional activities related to job responsibilities and career goals. Keeps current with social work and health care developments pertinent to job and profession. - Responsible for own professional growth and maintenance of required credentials. - Takes responsibilities for leadership and participation on committees and task forces of the Department of Medicine and the Department of Psychiatry, and the general hospital as needed. - Maintains and updates clinical knowledge and skills based on current health care practice/research. QUALIFICATIONS 1. Master's of Social Work (MSW) from an accredited program required. 2. Massachusetts Licensed Independent Clinical Social Worker (LICSW) required. 3. Bilingual in Spanish preferred but not required. SKILLS/ ABILITIES/ COMPETENCIES REQUIRED - Strong assessment and treatment skills, utilizing evidence-based approaches and protocols. - Excellent organizational skills. - Strong time management skills; ability to re-prioritize tasks as needed to meet demands with little supervision. - Demonstrated ability to communicate effectively orally and in writing. - Demonstrated ability to navigate electronic health records. - Strong interpersonal skills enabling effective team collaboration. - Demonstrated ability to understand the role of social worker in a community healthcare setting and to work in a complex, fast paced environment. WORKING CONDITIONS: - Able to provide assessment services in office or remotely using virtual technology. - Able to communicate orallyand in writing. - Able to document services provided using electronic medical record technology.
