LPN LEAD/ URGENT CARE CENTER/ 40 Hours/ Full Time/ Foxboro MA ABOUT THIS JOB DESCRIPTION The LPN Lead is responsible for the efficient flow of patient care, reporting directly to the Nurse Manager of the Foxboro Urgent Care / Multispecialty Clinics. Coordinates and oversees the activities of the LPN team within the multispecialty clinics to ensure compassionate, effective, and efficient delivery of high-quality clinical care for our patients. Operational Responsibilities 1. Develop and maintain staffing schedules. 2. Receive staff tardy/out calls and reassign staff as necessary. 3. Serve as a clinical back up in the event of a staffing need due to vacations, illnesses, lunches coverage, etc. 4. Organize and assign duties to clinical team members. 5. Oversight of medication inventory, expirations, and ordering. 6. Collaborates with inventory controller/CPD staff to ensure availability of supplies, instruments, and scopes. 7. Assure compliance with JCAHO regulatory requirements. Human Resource Responsibilities 0. Supervisory responsibility of up to 15 FTE’s. May include LPN’s, MA’s, Medication Support Coordinator. Varies with the needs of the practice. 1. Assemble and deliver performance evaluations to staff after 90 days and annually. 2. Participate and conduct disciplinary action as needed and in conjunction with the nurse manager. 3. Complete weekly payroll by deadline. 4. Respond to staff time off requests. 5. Monitor daily performance of clinical team members. 6. Assist with or conduct periodic staff meetings with team members. 7. Assist with interviews, hiring, and onboarding new staff members. Other Responsibilities 0. Participates and leads special projects as assigned by the Nurse Manager, maintains flexibility regarding workload and the needs of the department. 1. Attend periodic trainings and meetings pertinent to position. 2. Must complete the BWH POCT SME training within 6 months of hire. 3. Maintain excellent communication and effective working relationships with patients, physicians and/or APPs, clinical and administrative team members. GENERAL OVERVIEW: The LPN Lead is a valued member of the clinical leadership team. The primary responsibility of the LPN Lead is to coordinate and expedite the clinical operations of a multi-disciplinary team, ensuring an exceptional patient experience. The LPN Lead is accountable for patient care as assigned by a licensed independent provider (LIP: MD, NP, PA) or a Registered Nurse (RN). The LPN Lead participates in direct and indirect nursing care including procedures, health maintenance, patient instruction, collaborative planning and rehabilitation within scope of practice and consistent with his/her education and training. Developing relationships with patients, families and colleagues, the LPN Lead functions as an interdisciplinary team member who provides quality patient care and contributes toward the attainment of goals for the patient, the practice and the organization. The LPN Lead is accountable for facilitating an appropriate response to the access needs of patients in alignment with practice-based resources. The LPN Lead has leadership oversight of Medical Assistants (MA) and Licensed Practical Nurses (LPN) as determined by, and under the direction of, the hiring Department. PRINCIPAL DUTIES AND RESPONSIBILITIES: Leadership: Coordinate and expedite clinical operations including, but not limited to: 0. Functions within and across interdisciplinary teams in a manner that promotes coordination, mutual respect and timely response to the patient's health care needs. 1. Together with Nursing Leadership (Nurse Director and Professional Development Manager), and Practice Interprofessional team, develop patient education material. 2. Together with Nursing Leadership (Nurse Director and Professional Development Manager), and Practice Interprofessional team, develop telephone screening appropriate for clinical scenarios in hiring department, and establish interprofessional team-based care approach for nursing. 3. Facilitates development of clinical workflows and pathways that ensure the coordination of care. 4. Facilitates huddles, promotes and models effective, respectful communication. 5. Utilizes a critical thinking approach to problem solve and make decisions. 6. Identifies and evaluates operational clinical needs and resolves concerns early. 7. Manages all resources to support optimal care of patients seeking services in the clinic, telephonically and electronically including; Gateway, telehealth, and others. 8. Models and promotes health care team building and leadership skills. 9. Occasionally participates on committees or broader organizational initiatives as required. Management 0. Provides clinical oversight and support as needed to LPN nursing staff and medical assistants in collaboration with the Medical Director, Nursing Director, Practice Manager(s), and Practice Administrator. 1. Manages changes in LPN /MA staffing schedule and appropriately adjusts resources to ensure patient care and operational needs of the practice are met. 2. Provides feedback for evaluation of performance of LPNs and MAs under the delegation/direction of the Practice Manager and / or in collaboration with the Nurse Director. 3. Participates in the selection, supervision of LPN and MA team, per department structure. Mentors new staff assists with orientation and staff development needs. Quality and Safety 0. Collaborates with nursing and practice leadership in setting quality standards for nursing care and in the development of relevant policies and procedures. 1. Demonstrates ability to analyze, synthesize and apply data to drive optimal care delivery outcomes. 2. Oversight for TJC readiness and quality programs. Clinical Care 0. Provides evidence-based, clinically competent nursing care in a culturally sensitive manner. 1. Performs all clinical tasks and responsibilities of the BWH LPN as permitted by licensure, scope and institutional policy, and demonstrates operational knowledge of procedures and equipment. 2. Ability to accurately perform vital signs and obtain pertinent health information including but not limited to; temperature, pulse, respirations, blood pressure, height, and weight. 3. Ability to administer medication; routes of administration may include injections, intravenous and oral medications as per BWH Nursing Policy Med-01. 4. Ability to participate in the medication review process and related prescription management tasks, including prior authorizations. 5. Communication skills: Demonstrates effective oral and written communication skills. Communicates in a professional, courteous, clear and concise manner. Facilitates therapeutic relationships between patients, families and other members of the health care team. 6. Writing Skills: Ability to correspond and communicate with others clearly and effectively in writing and to take complete and accurate messages. 7. Phone skills: Ability to telephonically support the practice by answering and screening calls, offering information as appropriate and facilitating communication. 8. Organization Skills: Ability to manage work processes in a neat and orderly way. Ability to manage multiple tasks effectively, follow established protocols, and work within systems. Technology 0. Performs in the role of an Epic super user. 1. Actively participates in Nursing Informatics. 2. Demonstrates technological literacy in using computerized information systems. Physical (add to addendum B) 0. Ability to stand and walk for prolonged periods, climb stairs, bend, stretch, lift and have adequate (baseline) sensory functions: sight, smell, hearing and manual dexterity. Primary Responsibilities (80%): 0. Performs all patient phone calls in accordance with Department standards and scope of practice. 1. Screen patient calls for urgency of medical treatment and suggest review and recommendations from LIP. 2. Post-op phone calls 24 hours after surgical appointment. 3. Call or fax pharmacies with appropriate medication prescriptions and renewals as directed by the LIP. Including the completion of prior authorization forms. 4. Responsible for discussing normal lab and pathology results with patients as directed by LIP. Assist/set up any further appointments/testing required/requested by LIP. 5. Performs and documents all patient interaction, lab results, and prescription refills in EPIC and in accordance with BWH and Department procedures. 6. Monitor Gateway messages. Respond/forward to LIP as appropriate. 7. Maintain/update screening reference material. Secondary Responsibilities (20%): 0. Working with Nursing Leadership (Nurse Director and Professional Development Manager) to develop patient education material. 1. Working with Nursing Leadership (Nurse Director and Professional Development Manager) to develop telephone screening appropriate for clinical scenarios in hiring department, and establish interprofessional team based care approach for nursing. 2. Attend clinical management meetings, including but not limited to being lead on nursing meetings. 3. Reviews and communicate chart reviews for LIPs as well as clinical team members, including LPNs and Medical assistants. 4. Accurately performs patient intake, using two patient identifiers. 5. Responsible for procedure set-up and LIP assistance for procedures, using sterile technique. 6. Responsible for stocking inventory of all equipment and supplies used for the delivery of clinical care, and ensuring supplies are ordered and available. 7. All other duties as assigned, including cross-coverage to front office, and travel to satellite practices when necessary. QUALIFICATIONS: Education: - Graduate of an accredited Licensed Practical Nursing Program. - Current LPN Licensure in the Commonwealth of Massachusetts. Experience: 1. Recent (within 5 years) experience in the ambulatory setting as an LPN required. 2. Recent (within 5 years) demonstrated experience in specialty-specific clinical practices, workflows, and operations strongly preferred. 3. Recent (within 5 years) Leadership experience preferred. 4. Prior supervisory experience preferred. Skills and Abilities: - Must exercise a high degree of professional judgment within the scope of licensure - Possess interpersonal skills to interact effectively across roles, teams and departments. - Possess ability to manage critical issues relating to patient care and changing organizational climate. - Continually updates clinical knowledge and skills through formal and informal education and review of the current literature. - Must work well independently, have sound decision making skills and work effectively with and through inter-professional colleagues when required to make and facilitate complex decisions. - Excellent communication skills and strong team player. WORKING CONDITIONS:. General Requirements: Hospital based ambulatory practice. Normal patient environments where there are some exposures to communicable diseases, unpleasant odors, needle and blood products. Schedule Requirements: - Flexible Shift eligible - Alternate Work Schedule is not available for this position SUPERVISORY RESPONSIBILITY: - Medical Assistants - Licensed Practical Nurses HOSPITAL WIDE RESPONSIBILITIES: Works within legal, regulatory, accreditation and ethical practice standards relevant to the position and as established by BWH/Partners; follows safe practices required for the position; complies with appropriate BWH and Partners policies and procedures; fulfills any training required by BWH and/or Partners, as appropriate; brings potential matters of non-compliance to the attention of the supervisor or other appropriate hospital staff.
