Great opportunity to join an experienced Case Management team. Role focuses on transition planning for our high risk population SW Case Managers are assigned geographically to inpatient units and work on consult basis Support staff includes a Community Health worker to connect clients to community services Looking for good critical thinking skills and willingness to learn in a fast paced environment. Culturally competent. - Three or more years of clinical experience in a health care delivery system. - Completion of an accredited masters level SW program. - Current SW License in the State of MA is required. Preferred: - Case Management, Home Care Discharge Planning experience
