Schedule: Full Time (40 hours) The Substance Use Disorder (SUD) Clinical Social Worker is a key member of the interdisciplinary team who provides and oversees the provision of psychosocial care for selected patients with the focus on substance use disorders and the impact of the disorder on a patient and their families. This role provides counseling regarding potential options for resources/treatment/referrals for patients in the ED and inpatient medical floors with substance use disorders including alcohol use disorders and /or other drugs. As such, the social worker will conduct bio-psychosocial assessments and provide interventions and treatment as indicated. The social worker will be able to identify high risk psychosocial factors that impact the health status of the patient and help the interdisciplinary team members understand the influence of those factors upon the course of the patient’s medical care. The social worker will be able to determine when the complex psychosocial factors may warrant additional intervention and planning (homelessness, housing instability, mandated reporting/protective services, community violence, intimate partner violence, psychiatric disorders, and the substance use disorders as well). Clinical interventions will be in supportive collaboration with the patient and/or family when indicated while insuring understanding of cultural, age specific, environmental, emotional, physical, medical, and general well-being of the patient. Consults for the SUD Social Worker are requested through our inpatient Addiction Recovery Consult Service (ARCS) and/or through the ED and inpatient social work consults placed which specifically request SUD consultation for patients in the ED and on the inpatient med/surg floors. This position will collaborate with the ARCS providers, ED providers, and inpatient med/surg providers along with the ED and inpatient social workers and other members of the interdisciplinary team. The SUD Clinical Social Worker will participate as a member of the inpatient social work team and report to the inpatient social work manager. PRINCIPLE DUTIES AND RESPONSIBILITIES: - Provides bio-psychosocial assessment of patients to evaluate addiction/behavioral health history/emotional issues/coping style, understanding of illness/adjustment/compliance, barriers to care, cultural issues, abuse/neglect and domestic violence. - Provides psychosocial assessment of families to determine family relationships/systems as they relate to care of the patient. Identifies family decision makers and caregivers; family understanding of illness and trajectory of care. Identifies family coping style, family resources and cultural issues. - Employs a range of clinical interventions such as individual or family treatment, motivational interviewing, emotional supportive therapy, CBT, etc. Provides psychoeducation and caregiver/family counseling/support to promote family cohesiveness to provide care to patient coping with SUD challenges and/or mental illness. - Advocates on behalf of patients to gain access to services and resources. - Provides referrals for SUD treatment programs as part of discharge plan based on patient collaboration, preference, and agreement with patient signed 42 CFR for each referral. - Provides mandated assessments and filing of appropriate agency report if indicated when abuse is suspected (child, disabled adult, elder, institutional) and safety assessment when domestic violence is reported. - Identifies patients’ psychosocial, financial, legal, psychiatric or substance use that effect patient care management and collaborates with the team to facilitate patient care process. - Works effectively as part of the interdisciplinary health care team, communicating regularly with the team and other members on cases and as issues arise. Documents timely and relevant information. - Coordinates family/team meetings, as needed and/or if/when appropriate. Provides psychosocial consultation on patient care planning and patient/family management and community resources. - Facilitates the appropriate and efficient use of hospital and community resources. - Participates in formal and informal clinical case reviews, clinical supervision, and educational seminars. Serves on hospital wide committees as indicated. - Any additional duties as assigned. Our employees call it home. - Master’s Degree in Social Work from an accredited program. - Current professional licensure in Massachusetts (LCSW required, LICSW preferred). - Minimum of one year experience working with substance use disorders and effective treatment planning and discharge planning. - Internship/field placements in substance use treatment programs considered. - Knowledge of specific medical/psychiatric illnesses, procedures and treatments. - Excellent interpersonal skills, including negotiation skills necessary to work within a team. - Ability to provide rapid clinical psychosocial assessments and interventions with individuals, families, couples and/or groups. - Advanced crisis intervention skills. - Ability to work effectively as a member of a multi-disciplinary team - Knowledge of community agencies/resources.
