ChristianaCare HomeHealth is seeking a Part Time 20 hours a week RN Liaison. As a Licensed Practical Nurse Liaison for ChristianaCare HomeHealth you will identify home care needs of patients to ensure a smooth transition from acute care and/or alternative settings. The Nurse Liaison supports the intake process by interacting with discharge planners, case managers and patients.
Requirements:
- LPN license and eligible to work in Delaware
- 1 year experience in home health, hospice or care coordination preferred
- Ability to work and travel to post acute care settings
