Description

Work Schedule: 

70%, day shift. Eight hour shifts ranging from 7:00AM – 7:00PM Monday – Friday, and 7:00AM – 3:30PM on Saturdays. You will work at University Hospital and Eastpark Medical Center in Madison, WI. Hours may vary based on the operational needs of the department.

We are seeking a Clinical Pharmacist – Non-Oncology Infusion and Clinics:

  • For non-oncology infusions administered across UW Health Infusion Centers (e.g. rheumatology, gastroenterology, neurology) as well as specialty clinic administered medications across UW Health clinics:
    • Review appropriateness of dose, route, rate, timing and frequency of administration and indication for all medications.
    • Ensure drug regimens are consistent with medication use guidelines and policies and/or other evidence-based guidelines, and medication formulary.
    • Adjust regimens based on approved protocols and provider consultations.
    • Perform targeted or therapeutic drug monitoring and pharmacokinetics, and adjustment of drug regimens, if necessary.
    • Review for disease progression and resolution of symptoms, and consult with providers if treatment goals and/or resolution of symptoms are not achieved.
  • Provide training and education to pharmacy residents and PharmD students through practical and didactic experiences 

The ideal candidate will have experience focused in ambulatory care or health system setting, is self-motivated, and excited to start pharmacy services in this new specialty area! BCPS or BCACP is preferred but not required for hire.

  Qualifications
  • Bachelor's Degree in Pharmacy or a PharmD degree Required
  • Completion of an ASHP Accredited Residency training program Preferred
  Work Experience
  • 1 year of clinical patient care and orders management experience Required
  • 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred
Licenses & Certifications
  • Licensed to practice pharmacy in the State of Wisconsin or must obtain within 90 days of hire date Required
  • BLS within 90 days Required
  • Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) Preferred
