Summary:
Provide initial and comprehensive skills and education to children with diabetes and their families regarding planning, coordinating, implementing, and evaluating self-managed diabetes care. Interacts with and confers with medical providers and diabetes team members in evaluating and developing child and family's knowledge and skills in diabetes care.

Responsibilities:
* Patient Care: Conducts focused patient interview and assessment based on the reason for the visit and confers with physicians, staff, diabetes care team in providing medical management of diabetes care. Collects, assesses and reviews patient data (laboratory tests, medical history, diagnostic procedural results, etc) and confers with physicians, staff, diabetes care team in providing medical management of diabetes care. If patient presents with unstable clinical findings, a complete thorough physical assessment will be conducted.
* Patient education: Collaborate with patient, family and diabetes care team to establish goals for diabetes self-management skills training and education. Provide initial and continuing education and skills training for children and families on diabetes care, both in-patient and outpatient. Educate patient and family with diabetes management plans and management of hypo- and hyperglycemia, ketosis, and sick days. Provide hands-on assistance to patient and family to ensure proper medication use, techniques and adjustments. Document patient/family learning needs assessment, goals, management plans, and understanding of education provided. Utilize licensed independent practitioners, community resources and any other resources, as indicated.
* Quality/PI/Regulatory: Comply with state, federal and other regulations. Participate in obtaining requirements for ongoing national program certification and recognition. Maintain patient safety, privacy and confidentiality.
* Telephone Triage: Patient and family phone calls are triaged by the diabetes care team based on the reason for the call. Non urgent calls are handled by the diabetes care team and may consist of (but are not limited to) assistance with diabetes management plans, glucose and/or food logs, and/or community care resources. Urgent sick calls are handled by a registered nurse. Telephone assessment is guided by telehealth guidelines and nursing judgment.
* Trains, assists and/or mentors employees with less experience, skill or tenure
* Perform other duties and special projects as assigned

 How You’ll Be Successful:

WORK EXPERIENCE
* At least 2 years experience working as a registered nurse Required
* Pediatric Nursing Experience Preferred
* Experience as a diabetes educator in a pediatric setting Preferred

EDUCATION
* Four-year Bachelor's degree in Nursing (BSN) Required
* Education required to maintain CDE certification/eligibility Required

LICENSES AND CERTIFICATIONS
* Current license to practice professional nursing in the State of Texas Required
* BLS certification approved by the American Heart Association Required
* Certified as a Diabetes Educator (CDE) by the National Certification Board for Diabetes Educators or eligible to take exam once necessary experience is obtained. Must obtain certification within five (5) years from date of hire. Required
* Effective April 20, 2020, new employees must obtain PACES certification within 6 months of hire and existing employees must obtain PACES certification upon expiration of their PEARS certification (no later than April 20, 2022). Req
