Position Overview:

**$5,000 Sign-on bonus available to external candidates - in accordance with Sutter Health policy**

Under the supervision of the Clinical Manager, the Primary Care Clinician (PCC) is an experienced physical therapist who is responsible for the overall case management of home health patients including the provision of direct care with a designated visit productivity standard. This position collaborates, plans, organizes, and directs all patient care services for patients in the assigned caseload in accordance with current standards and regulations, ensuring the optimal degree of quality care is maintained in coordination with an interdisciplinary team.
Implements the established plan of patient care and ensuring effective management of visit utilization and control of expenses.
Responsible for assessing, planning, organizing, and participating in rehabilitative programs that improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.
Demonstrates advanced knowledge and practice level competencies, and is a recognized clinical or workflow subject matter expert.

Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care.

May assist in agency wide orientation, competency evaluations and consultation to clinical staff related to specific expertise as requested by Clinical Manager.

The Primary Care Clinician reports directly to the Clinical Manager as do the other members of the interdisciplinary team. The Primary Care Clinician may give functional guidance to Team RNs, LVNs, PTAs, OTs, COTAs, SLPs, MSWs, RDs and HHAs.

Job Description:

EDUCATION

• Graduate of an accredited physical therapy school.

CERTIFICATION & LICENSURE

• California Physical Therapist License
• BLS-Basic Life Support Healthcare Provider - Issued by the AHA
• DL-California Drivers Class C
• Automobile Insurance

PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:

• 1 years experience in primary work assignment area.

• Critical thinking, complex problems solving, decisive judgement and ability to work independently.
• Knowledge and application of professional practice and regulatory requirements.
• Must be able to work in demanding work environment, organize multiple priorities completing work duties within expected timelines and requirements.

• Demonstrates effective and efficient professional communication (verbal & written) skills as well as interpersonal skills, conflict resolution, teamwork/collaboration, customer service and community relations abilities.

• Ability to develop training materials and competency assessments.

Pay Range is $68.10 to $85.11 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
