Job Summary and Qualifications

We are seeking a Registered Nurse Patient Navigator. You will guide patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. What you will do in this role

• You will coordinate and monitor patient care and progression through the Care Continuum.
• You serve as a single point of contact for referring physicians, patients and caregivers to ensure access to clinical and support care services.
• You assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies.
• You develop patient education programs and tools, providing appropriate teaching, outreach, and education to patients and families.

 

Requirements:

• Licensure as a Florida Registered Nurse in accordance with state regulations or appropriate compact licensure. If compact license held, active FL RN license required within 60 days of hire.
• Bachelor’s Degree in Nursing or related health care field; or degree in progress.
• Minimum of one year clinical specialty area experience required.
• Current American Heart Association BLS (Basic Life Support) certification required.
• Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills
