Job Description

The primary functions of the Registered Nurse (RN) include administering skilled nursing care for clients of all ages in their place of residence, coordinating care with the interdisciplinary team, patients/families, and referring agencies.

Essential Functions

1. Assess home health and/or hospice patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs.
2. Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
3. Collect data on physical, psychological, social, spiritual, and cultural factors influencing patient/family health status and incorporate this data into the POC.
4. Maintain up-to-date patient records, ensuring all visits, contacts, problems, plans, actions, and goals are accurately documented.
5. Provide care utilizing infection control measures to protect both staff and patients.
6. Educate patients and/or their families regarding care, diet, medications, and home safety, using appropriate teaching methods.
7. Supervise and provide clinical direction to Home Health Aides (HHA), State Tested Nursing Assistants (STNA), and Licensed Vocational Nurses (LVN).
8. Assess and reassess pain, utilizing appropriate pain management techniques and educating patients and families on pain management.
9. Arrange referrals to appropriate agencies as needed.
10. Monitor assigned cases to ensure compliance with third-party payer requirements.
11. Perform on-call responsibilities and provide services to patients/families as assigned.
12. Serve as a liaison to patients/families, team members, administration, physicians, and other healthcare professionals.
13. Attend interdisciplinary team meetings and other necessary meetings.
14. Perform other related duties as assigned.

• Competence in venipuncture, IV care, and routine medical/surgical nursing skills.

• Flexibility to work a varied schedule and travel locally.
