Job details

The Medical Social Worker is responsible for supporting the social and emotional wellbeing of patients at Satellite Healthcare.
Conducts psychosocial assessments, participates in care planning and team review of the patient's current psychosocial needs.
Provides management of services to assist patients in coping with issues associated with chronic kidney disease and dialysis treatments.

Job requirements

Master’s Degree in Social Work is required.
One year related experience preferred for MSW I.
Three years minimum related experience or LCSW for MSW II.
Five years related experience with three at SHC or LCSW and three years SHC experience for MSW III.

Additional Details

Able to read, write, speak, understand and satisfactorily communicate with others in English in person, over the phone and via email.
