Description

CVHCare is seeking a qualified Occupational Therapist. New graduates are welcome to apply. Home health experience is preferred but not required.

Assigned Territory

  • Area #1: Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City
  • Area #2: Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont (choose one)
  • Area #3: San Ramon, Dublin, Pleasanton, Sunol, Livermore (choose one)

Work Options

  • Guaranteed Hours:

    • Minimum productivity of 27 points per week.
    • 36 hours guaranteed.
    • Must complete 30 points in the week.
    • Willingness to travel outside the area or work weekends is required.
    • Guarantee does not apply for the first and last week of the assignment, during holidays, or if time off is taken.
  • Non-Guaranteed Hours:

    • Weekend coverage may be required.
    • May need to go outside the service area to secure a full caseload.
    • Can refuse patients.
    • No guaranteed hours.
    • Must be willing to see Covid+ and Covid PUI patients.

Points and Hours Calculation

  • OASIS ReCertification, Routine, Evaluation, Discharge, and other visit types convert to points and hours as follows:
    • 1 point = 1.33 hours.

Credentials Required

  • Current California Occupational Therapist License
  • BLS Certification
  • Valid Driver's License
  • Auto Insurance
  • Auto Registration
  • Annual TB Test
  • MMR, Varicella, HepB, TDap (10 years), and Annual Flu Vaccination
  • Drug Screen
  • Physical Examination

Additional Information

  • All requested time off (RTO) must be disclosed at the time of submission.
  • Agencies must ensure their contract mirrors the confirmation page to avoid removal from the client’s vendor set.
