  • Contract Length: 13 weeks

Job Description

  • Must have a valid Massachusetts nursing license.
  • Minimum of 1 year of experience in Case Management, specifically in hospital discharge planning (experience in rehabilitation hospitals is a plus).
  • Familiarity with EMR and documentation expectations for the role (ACMA SOP).
  • Knowledge of Continuity of Care / Transition of Care requirements (CMS standards).

Shift Information

  • Schedule: Monday to Friday, 8 AM to 5 PM (5x8 days). Any variations to the 8-hour shift must be discussed and approved by the manager.

Required Certifications and Competencies

  • Certifications:
    • ACLS (Advanced Cardiac Life Support)
    • BLS (Basic Life Support)
    • PALS (Pediatric Advanced Life Support)
    • National Tech Certification (if applicable)
  • Competencies:
    • Medication Test/Pharmacology Competency Exam
    • General Competency Exam

Health Documents

  • Physical exam within 1 year of start.
  • Negative TB test or Chest X-Ray within 1 year of start.
  • TB Questionnaire.
  • Vaccination documentation for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella (Chicken Pox)
  • 10 Panel Drug Screen within 1 year of start.
  • Flu Seasonal Vaccination/Documentation (August - April).
  • COVID-19 Vaccination (Dose 1 and Dose 2 if applicable).

Background and Reference Checks

  • Standard background check (7-year search).
  • Annual checks for EPLS/GSA/SAM and HHS/OIG.
  • 2 Professional References required (within 2 years of start).

Additional Information

  • MA Nurse Aide Registry Search is required for all travelers in Massachusetts.
  • Massachusetts Statewide iCORI Clearance (Attestation) is mandatory for all candidates working at MA facilities.
