Job Description

• Role: Case Manager
• Patient Load: 20-30 patients per week
• Patient Conditions:

  • CHF (Congestive Heart Failure)
  • COPD (Chronic Obstructive Pulmonary Disease)
  • Pneumonia
  • Cancer
  • Dementia
  • Kidney Disease
  • Hypertension
  • Falls and Fractures
  • Infections
  • Cellulitis
  • Post-surgical care
  • Wound care
  • Ostomy care
  • Labs
  • IV infusion

Support Team

• Manager/Supervisor
• Lead
• Scheduler

Requirements

• Experience: Minimum of 1 year in Home Health as a Registered Nurse (RN)
• Licenses/Certifications:

  • Active state RN license
  • BLS (Basic Life Support) certification (AHA)

Skills Required

• Wound care (including wound vacs, pleural drains, ostomy care)
• Peripheral blood draws
• Central line care
• IV administration through multiple devices (including elastomeric and IV pumps)
• Excellent assessment skills
• Experience with computer documentation (EPIC)

Additional Information

• Scrubs: Any color acceptable, must be appropriate/professional
• Equipment: Laptop, Sapphire IV pumps
• Transportation: Traveler must provide their own transportation to/from all patient visits.
