Job Description

• Role: Home Health LPN
• Patient Ratios: 1:1 (may visit up to 6 patients per day; 30 productivity points standard)
• Experience Required: 1 year of skilled home health experience
• Certifications Required: BCLS
• On-Call Requirement: No
• Floating Requirement: No
• Weekend Requirement: No
• Local Candidates Accepted: Yes (within 50-75 mile radius)

• Guaranteed Hours: Facility can call off up to one shift per pay period

Health Documents Required

• Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO.
• TB Test: IGRA/TSPOT, QGold, or 2 PPDs within 12 months of start date.
• Vaccinations: Tdap, MMR, Varicella, Hepatitis B, Flu Vaccine, COVID-19 Vaccination.
• Drug Testing: 10 Panel Drug Screen within 90 days of start.

Background Check

• Required: Background check covering all locations lived/worked within the past 7 years, updated annually.

References

• Required: Minimum of 2 references covering 1 year of employment total within the last three years.
