Job Opportunity: Registered Nurse - Discharge Planner Case Manager
Location
- City: Washington, DC
- Facility: Medstar Washington Hospital Center
- Hospital System: MedStar Health
Employment Details
- Type: Travel/Contract
- Contract Length: 13 weeks
- Shifts: 8:00 AM - 4:30 PM, 5 days a week
- Weekend Requirement: Included in rotation (every 5th or 6th weekend); no on-call requirement
- Floating Requirement: None
Job Description
The role involves working as a Discharge Planner Case Manager, focusing on discharge planning, care coordination, and conducting needs assessments.
Patient Population
- Unit Patient Diagnosis: Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, MICU
Requirements
- Licensure:
- DC RN Licensure (will accept pending DC license with proof of application)
- Alternatively, candidates can hold LMSW/LGSW/LCSW
- Certifications:
- BLS (Basic Life Support)
- Completion of at least 1 travel assignment
- Experience:
- Minimum of 2 years in discharge planning, including:
- Admission criteria review
- Benefits eligibility
- Care coordination
- Conducting needs assessments
- Ordering DME
- Experience with home health, hospice placement, LTAC, nursing home placement, rehab placement, and acute hospital experience
- Prior charge experience preferred
- Regulatory Skills: Knowledge of CMS, HIPAA, JCAHO; workers' compensation experience preferred
Additional Information
- Charting System: Cerner
- Uniform: Business casual; no scrubs required
- Vaccination Requirement: All employees must have the Flu vaccine
Health and Safety Documents
- Physical Exam: Required upon hire
- TB Testing: IGRA (T-spot, Quantiferon Gold) or PPD Skin Test required annually
- Vaccinations:
- Tdap within 10 years
- Hepatitis B Titer or signed declination
- Varicella Vaccines or Positive Titer
- COVID-19 Vaccination (Dose 1 and 2 if applicable)
Background and History Checks
- Criminal Background Check: Required within 90 days of start
- References: Two positive supervisory references required
