Job Description

Marysville Post Acute is seeking a dedicated and experienced Physical Therapist to join their rehabilitation team. The facility is known for providing comprehensive rehabilitation and skilled nursing care, focusing on personalized, compassionate care.

Key Responsibilities

• Conduct thorough assessments of patients' physical conditions and functional limitations.
• Develop individualized treatment plans based on patient assessments and goals.
• Implement physical therapy interventions, including exercises, manual therapy, and modalities.
• Educate patients and their families on treatment plans and home exercises.
• Monitor and document patients' progress, adjusting treatment plans as needed.
• Collaborate with interdisciplinary team members to ensure coordinated care.
• Participate in team meetings and continuing education opportunities.
• Maintain accurate documentation of patient care in compliance with standards.
• Adhere to ethical and professional standards set by the American Physical Therapy Association (APTA) and state licensing board.

Qualifications

• Doctor of Physical Therapy (DPT) degree from an accredited program.
• Current state licensure or eligibility for licensure as a Physical Therapist in California.
• Proven experience as a Physical Therapist, preferably in a post-acute or rehabilitation setting.
• Strong clinical skills in assessment, treatment planning, and implementation.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively.
• Commitment to ongoing professional development.
• CPR certification (preferred).

Physical Requirements

• Ability to lift and move patients, equipment, and supplies.
• Prolonged periods of standing, walking, and performing physical therapy interventions.
• Manual dexterity and strength to perform therapeutic exercises and manual therapy techniques.
