Job Description

The Community Health Nurse (CHN) provides, teaches, supervises, and documents nursing care for the sick and injured at home. Key responsibilities include:

• Promoting, maintaining, or restoring health to prevent illness and facilitate rehabilitation.
• Collaborating with other healthcare providers and community representatives to coordinate home care.
• Evaluating individual and family responses to interventions.
• Functioning independently under general guidance and following specific guidelines and protocols.

Required Skills and Qualifications

• Experience:

  • One year of acute nursing care required; hospital-based experience preferred.
  • Experience in home health is required.
• Certifications:

  • Certified Home Health required, including proficiency with OASIS.
  • Infusion Therapy Certification preferred.
• Education:

  • BSN strongly preferred.
• Other Requirements:

  • Current Driver License and insurance required.
  • Ability to drive in inclement weather.
  • Must have a vehicle for home care visits.
  • Ability to exercise independent judgment and demonstrate critical thinking skills.
  • Strong interpersonal skills and ability to maintain confidentiality.
  • Flexibility regarding work hours and willingness to accept "on call" responsibilities.

Additional Information
