Job Description

As a Home Health RN, you will be responsible for:

• Assessing patient conditions
• Implementing care plans
• Providing ongoing education and support to patients and their families
• Delivering patient care in accordance with organizational philosophy, policy, and community health practice standards
• Performing admission processes and case management responsibilities
• Utilizing thorough and timely electronic documentation and interdisciplinary communications

Requirements

• Experience:

  • 1 year of Certified Home Care experience
  • 1 year of Case Management experience
• Travel: Ability to travel to assigned cases
• Weekend Rotations: Once every 6 weeks

Credentialing Requirements

Mandatory Credentials

• MGB Badge Photo: Submit a color photo taken in the past 6 months.
• MGB MVR Check: Motor Vehicle Record check required for all home care positions.
• License/Certificate Verification: Primary source verification of professional license and/or certification.
• Fit Test: N95 fit test required within 365 days of start date.
• MGB Nurse Aide Registry Check: Required by MA DPH for all employees.
• COVID-19 Vaccine: Completed primary vaccination series required.
• MMR Vaccination: Documentation of measles, mumps, and rubella vaccinations required.
• Influenza Vaccine: Required during flu season (Sept 15 - April 30).
• TB Test: Documentation of TB skin test or QuantiFERON TB Test required.
• Health Statement: Physician statement signed by MD/DO/NP/PA required.
• MGB Criminal Background Check: Required within 45 days prior to start date.
• MGB Massachusetts CORI: Criminal history check required.
• MGB Resume: Include past 7 years of work experience.
• MGB References: 2 recent references required, including at least 1 supervisor.
• MGB Skills Checklist: Required for submission.
• MGB Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.
• Education Verification: Verification of highest level of education achieved.

Preferred Credentials

• Hepatitis B: Record of vaccinations or proof of immunity preferred.
• Tdap: Vaccine within 10 years preferred.
• Positive PPD - Negative Chest X-Ray: Required if positive on a TB skin test.

Additional Information

• All credentials must be submitted in accordance with the facility's policies and standards.
• Documentation must be updated as required by the facility and state regulations.
