Job Opportunity: Outpatient/Inpatient Case Manager/Utilization Review Registered Nurse (CM/UR RN)

Job Description
The CM/UR RN is responsible for:
- Direct supervision and supportive contact for identified patient assignments and high-risk patients.
- Designing and managing a continuum of care to empower clients towards self-sufficiency.
- Conducting assessments, service planning, resource acquisition, and monitoring client progress.
- Performing Utilization Review and Management/Clinical Documentation Chart Reviews in various care settings.
- Collaborating with Social Work Case Managers on discharge planning and care transitions.
- Linking clients with community social service providers and healthcare resources.
Qualifications
Education
- Associate's Degree in Nursing (ADN) required; must obtain a Bachelor's in Nursing within two years of hire.
License/Certification
- Current Basic Life Support (BLS) certification by the American Heart Association (AHA) required.
- Valid, unrestricted Professional Nursing License to practice as a Registered Nurse (RN) in any U.S. state or territory.
Experience
- Minimum of three years of clinical nursing experience with at least two years in an inpatient setting (medical-surgical unit, PACU, ICU, etc.) or home health.
- Knowledge of case management, purchased referred care, and utilization review processes.
- Familiarity with electronic health record systems.
- At least one year of traveling experience as a contractor is required.
Additional Information
- Orientation: Every two weeks.
- COVID Exemptions: No religious exemptions accepted; medical exemptions require approval from the medical review board.
- Housing Application: Must be submitted two weeks prior to the start date.
