Job Description

The Mammographer will perform screening and diagnostic imaging.

Required Credentials

1. License/Certificate Verification

   • Primary Source Verification of professional license and/or certification required within 30 days of start date. MA Conditional Licenses not accepted.

2. ARRT Certification

   • Required for onboarding.

3. COVID-19 Vaccine

   • A completed primary COVID-19 vaccination series approved by the FDA or WHO is required. No exemptions or waivers will be considered.

4. MMR Vaccination

   • Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine, or proof of immunity via blood test.

5. Influenza Vaccine

   • Required during flu season (September 15th - April 30th). No exemptions or waivers will be considered.

6. TB Test

   • Documentation of TB skin test screening within 3 months of start date or a negative QuantiFERON TB Test.

7. Health Statement

   • Physician statement signed by MD/DO/NP/PA required within 365 days of start date.

8. MGB Criminal Background Check

   • Must include all counties resided, employed, and attended school within the last 7 years.

9. MGB Massachusetts CORI

   • Criminal history check for the state of MA going back to age 17.

10. MGB Resume

    • Include past 7 years of work experience and education.

11. Respirator Medical Clearance (RMC) Attestation

    • Required prior to being fit tested.

12. MGB References

    • Two recent references, including at least one supervisor or above.

13. MGB Skills Checklist

    • Required for submission.

14. MGB Confirmation of Assignment/POI Form

    • Required for all initial assignments and extensions.

15. Education Verification

    • Verification of highest level of education achieved.

16. MGB SSN Trace

    • Must list addresses/aliases for the past 7 years.

17. MGB National Sex Offender Check

    • Conducted within 30 days of start and annually.

18. MGB Exclusion Checks

    • Search OIG, GSA, and MassHealth & NH Medicaid exclusion lists.

19. MGB TB Symptom Survey (if Positive PPD)

    • Must be completed prior to start and updated annually.

20. MGB Submission Checklist

    • Required for submission.

Additional Information

• Strongly preferred: Documentation of 3 Hepatitis B vaccines or proof of immunity via blood test.
• Documentation must be uploaded as PDF where specified.
• All credentials must be updated if there is a break in service greater than 6 months.
