Job Opportunity: Occupational Therapist (OT)

Overview

  • Contract Length: 13 weeks

Job Description

  • Candidates with an Arizona home address on their driver's license will not be considered.
  • All submissions must include a copy of the driver's license.
  • Candidates should be open to working in various departments and locations; please specify preferences in the submission.
  • All requests for time off (RTO) must be listed and cannot exceed 7 days. RTO will not be approved during the first week of assignment or on holidays.
  • Candidates must be pre-closed and ready to accept the position.

Requirements

  • Experience: Minimum of 24 months of current Home Health OT experience is required.
  • Certifications: Must have active certifications from the American Heart Association or American Red Cross included in the submission.
  • Background Check: No license hits are allowed.
  • Vaccination: Flu vaccination status must be included in submission notes. Banner declinations can be applied for.
  • Travel Experience: Preferred but not required.

Submission Requirements

  • Complete Banner Cover Page.
  • Updated resume detailing all facilities, facility size, beds, trauma level, and units.
  • Skills checklist specific to the discipline (must be less than a year old).
  • Active license verification (must be less than 30 days old).
  • Driver's license copy.

Additional Information

  • Learning modules must be completed prior to the start of the assignment.
  • Non-billable hours have been adjusted to reflect 16 hours for each assignment.
  • The client or vendor may cancel an assignment in progress with 7 days written notice without penalty.
