Overview

Shift Details

• Schedule: Monday to Friday, 9:00 AM - 5:30 PM
• On-call: No call, no autopsy
• Floating: Must be willing to float between local facilities as needed

Qualifications

• Education: Bachelor’s degree in health sciences
• Experience: Minimum of 1 year in a relevant health sciences role
• Certification: PA (ASCP) required

Required Credentials

1. License/Certificate Verification: Current state or compact license and Primary Source Verification completed prior to start.
2. COVID-19 Vaccine: Required; provide vaccination card with submission. Declinations accepted for medical or religious reasons.
3. MMR: Documentation of 2 MMRs at least 28 days apart and/or positive laboratory titer.
4. Fit Test: Completed onsite and annually.
5. Hepatitis B: Documentation of Hep B vaccinations (series of 3) and/or positive laboratory titer or signed declination.
6. Influenza Vaccine: Status required; declination reviewed for religious or medical exemption only.
7. TB Test: 2 negative TB skin tests (1 within 90 days and 1 within 365 days of start) or Quantiferon Gold within 90 days, updated annually.
8. Varicella: Documentation of 2 doses of varicella 4 weeks apart and/or positive laboratory titer.
9. Tdap: Proof of having received vaccine.
10. Physical/Health Statement: Written statement confirming physical capability to perform job functions, completed within one year prior to assignment.
11. Norton Profile: Full work experience with references from previous managers.
12. Norton Specialty Exam: Passing score of 80% or higher, updated every 3 years.
13. Norton Skills Checklist: Completed prior to start, updated annually.
14. Norton Drug Screen: Standard 10 panel drug screen, completed within 12 months before start and updated annually.
15. Norton References and Employment Verification: Completed prior to start, covering 1 year of employment within the last 3 years.
16. Norton Background Check: Comprehensive background check completed prior to start and updated every 3 years.
17. Norton Candidate Affirmation: Completed within 24 hours of offer acceptance.
18. Norton Employment Eligibility: E-Verify and supporting documentation completed prior to start.
19. Norton I-9: Completed with supporting documentation prior to start.
20. Norton Job Description: Must align with Norton's job description.
21. Norton Joint Commission and Regulatory Standards: Workplace Safety Module completed prior to start and updated annually.
22. Norton KY TB Risk Assessment Form: Completed 90 days prior to start, updated annually.
23. Norton Work History Form: Facility Form must be completed.
24. Norton Adult Caregiver Misconduct Registry: Registry check for specified settings, updated every 3 years.
25. Norton Exclusion Search: Completed internally.
