Job Description

• MRI-certified
• Experience with GE/Siemens 1.5/3T preferred
• Inpatient/Hospital experience required

Required Credentials

1. BLS Certification: AHA, Red Cross, CHSF, or Military accepted.
2. License/Certificate Verification: Primary source verification required within 30 days of start date.
3. Fit Test: N95 fit test required within 365 days prior to start date.
4. ARRT Certification: Required.
5. COVID-19 Vaccine: Completed primary series or up-to-date vaccination required.
6. MMR Vaccination: Documentation of two measles, two mumps, and one rubella vaccine or proof of immunity.
7. Hepatitis B: Record of 3 vaccines or proof of immunity (optional).
8. Influenza Vaccine: Required during flu season (Sept 15 - April 30).
9. TB Test: Documentation of TB skin test or negative QuantiFERON TB Test required within 3 months of start date.
10. Health Statement: Signed by MD/DO/NP/PA within 365 days of start date.
11. Tdap Vaccine: Preferred within 10 years.
12. Criminal Background Check: Required within 45 days prior to start date.
13. Massachusetts CORI: Required within 30 days prior to start date.
14. Resume: Must include past 7 years of work experience with city & state.
15. References: Two recent references, including at least one supervisor.
16. Skills Checklist: Required for submission.
17. Confirmation of Assignment/POI Form: Required upon acceptance of assignment.
18. Positive PPD - Negative Chest X-Ray: Required if positive on TB skin test.
19. Education Verification: Must verify highest level of education achieved.
20. SSN Trace: Required to list addresses/aliases for the past 7 years.
21. National Sex Offender Check: Required within 30 days of start.
22. Exclusion Checks: Required for OIG, GSA, and MassHealth & NH Medicaid exclusion lists.
23. TB Symptom Survey: Required if positive PPD.

Additional Information

• All required documentation must be submitted prior to the start date.
• No exemptions or waivers for vaccination requirements will be considered.
