Job Description

The Case Manager will be responsible for assessing, planning, facilitating, and advocating for options and services throughout the continuum of care from point of contact through discharge for assigned patients. The role emphasizes care coordination, communication, and collaboration with nursing, physicians, and other departments within the facility, as well as with insurers and post-acute service providers.

Key Responsibilities

• Conduct utilization review processes for patients as required.
• Manage discharge planning processes in accordance with hospital policies, standards of practice, and regulations.
• Promote quality and cost-effective outcomes through effective case management.

Required Qualifications

• Licensure: Current RN License (CCM preferred)
• Education: Bachelor’s Degree preferred
• Experience:

  • Minimum of one year in acute care or payer case management, utilization management, discharge planning, and resource management.
  • Minimum of three years of acute care nursing experience.
• Skills:

  • Proficiency with information systems and computer systems (MIDAS preferred).
  • Advanced understanding of utilization management, case management, healthcare financing, and regulatory implications of practice.
