- BLS
- IN state license
Certification details
- Monday - Friday
- Minimum of 1 year OT at Acute Hospital
- Manual Therapy, Lymphedema certifications/licenses preferred.
- Cerner Experience preferred
- Inpatient acute care rehab team of 19 professionals. Expect to see patients in all levels of acuity from ICU to progressive care units with diagnoses of CABG & other cardiac procedures, stroke, TBI, joint replacement, fracture repairs, respiratory illnesses, dementia, MS, Parkinson, ambulation, etc. OT is expected to evaluate, develop and follow POC that may include ADL training, therapeutic exercises, therapeutic activities to improve function and asses appropriateness of discharge disposition recommendations on daily basis.

Schedule information
- Valid Driver’s License or Passport accepted. Must be clear copy.
- AHA ONLY. RQI is acceptable. Please check the cost center to verify what certifications are required.
- HIPAA Acknowledgement required.
- Online Verification of License to include expiration date and any disciplinary actions (within 30 days before start date). Must be through primary source.
- Ishihara exam or online testing from Relias (or equivalent) is accepted.
- Photo for Badge required asap. Must be professional looking.
- The Look of Assurance packet must be sent to all candidates for review and initial via DocuSign prior to start.
- Complete facility specific forms as a priority.
- Proof of valid auto insurance required for all travelers under specialty.
