• Work week is Sunday – Saturday.
• Vendor fee is 5%
• OT, Call Back & Holiday: 1.3x bill rate
• On call: $6.00 (must arrive at Facility within 30 minutes of receiving call. 2 hours minimum call back period )
• Guaranteed Hours: Guaranteed minimum work week applies to all work weeks, including weeks during which orientation is provided, weeks with holidays and weeks during which there may be unit closures for all or any portion of such week, except that Client shall have the option to cancel up to three shifts per thirteen-week assignment without penalty as needed for low census or changes in staffing needs.
• Non bill orientation hours: 12 for all SSM facilities

RTO Restrictions: Please request RTO up front if possible. Travelers may be required to participate in holiday and weekend rotation.
Guaranteed Hours: If confirmed to 3x12-hour shifts SSM will guarantee a min. of 36 hours/ week. If confirmed to 4x12-hour shifts SSM will guarantee a min. of 48 hours/ week. If confirmed to shift lengths less than 12-hours SSM will guarantee a min. of 40 hours/week. If confirmed to shift lengths less than 12-hours SSM will guarantee a min. of 40 hours/ week. Please note: SSM may cancel call-off the following per 4-week period: Up to 8 hours for Candidates confirmed to 8-hour shifts. Up to 10 hours for Candidates confirmed to 10-hour shifts. Up to 12 hours for Candidates confirmed to 12-hour shifts.
Special Requirements: CVOR experience required. ECMO Left Ventricular Assist Device Mitral Valve Replacement Scrub Cardiovascular Surgery Scrub Vascular Surgery ASD/VSD Repair Tetralogy of Fallot Repair Transposition of the Great Arteries Repair Valve Repair/Replacement Ventricular Assist Device Implant Scrub Pedi Cardiovascular Surgery Heart Transplant Organ Donation Scrub Pedi Transplant Surgery Administration of Blood Products. **Must have experience scrubbing and circulating CVOR including pump cases open heart procedures and ECMO**
