The RN-Long Term Care provides direct and indirect nursing care in the nursing home setting, in accordance with established policies, procedures and protocols of the healthcare organization.

To perform this job, an individual must perform each essential function satisfactorily with or without reasonable accommodation.

·      Performs all aspects of care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.

·      Interacts professionally with residents/families; treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethical, and religious/spiritual needs of residents and their families.

·      Effectively and appropriately communicate with residents, physicians, family members and co-workers, demonstrating customer service excellence through application of positive language principles, appropriate response and use of constructive problem-solving techniques which foster positive outcomes.

·      Perform an appropriate assessment on all residents as related to the care requested and provided and reassessments as per policy.

·      Adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the resident and family regarding pain management.

·      Applies principles of sterile techniques and infection control procedures.

·      Assists residents with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.

·      Maintains accurate and continued nursing documentation including resident histories, conditions, care given, treatments, responses, and assessment of changes. Documentation meets current standards and policies.

·      Follows the six (6) medication rights and reduces the potential for medication errors.

·      Maintains current knowledge of medications and their correct administration based on age of the resident and their clinical condition.

·      Knowledge of the principles of growth and development over the life span and the skills necessary to provide age-appropriate care to the residents served.

·      Interpret data regarding the resident’s status in order to identify each resident’s age specific needs and provide care needed.

·      Establishes a plan of care which is appropriate to problems identified and involves the resident according to his or her capabilities and desires. Identifies and documents goals, anticipated progress, and plans for reevaluation.

·      Formulate an individualized plan of care, revise plan as indicated by the resident’s response to treatment and evaluate overall plan daily for effectiveness. Involves resident and family in the formation of the plan of care.

·      Communicates appropriately regarding changes in resident’s status, symptomatology, and results of diagnostic studies.

·      Functions as liaison between administration, residents, physicians, and other healthcare providers.

·      Assist physicians with procedures, as needed.

·      Gives a thorough report to oncoming shift by participating in walking rounds and documenting appropriately on calendar for upcoming needs.

·      Follow through as needed on information given by shift report, resident, or family regarding resident concerns.

·      Assists in orientation and training of new staff/students, as necessary.

·      Keeps current with principles and practices, regulations, and standards that apply to healthcare.

·      Performs other reasonably related job duties as assigned or requested, within scope of practice.  


Qualifications

Education:

Associate Degree in Nursing (ADN), Required

Bachelor of Science in Nursing (BSN), Preferred

Required Licenses:

Must have a current Mississippi Registered Nurse license or a current Compact State Registered Nurse license.

Required Certifications:

·      American Heart Association (AHA) Basic Life Support (BLS) Certification required within 90 days of hire or transfer.

   Required Skills:

Requirements will include successful completion of orientation checklist and skills/credentials/competency requirements of specific area of work.

·      Demonstrates the values of team concept on a consistent basis.

·      Focused on prioritizing the resident’s clinical experience, maintaining a high awareness of resident satisfaction.
