AMDIJP00009098 RN Home Health Description:
Provides coordinates and directs the provision of home nursing care according to physicians orders based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager Directly Supervises and Evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
DEGREE OF TRAVEL
Home visits. Periodic office meetings. Must have reliable transportation current drivers license and agency-required liability insurance.'
Average # of patients/day: 5-8
Points per day/week: How are points broken down 30 points per week. Each type of visit and activity (documentation travel etc.) hold a different point value. Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points.
Guaranteed Hours: No if there is a situation where the contractor is unable to meet their financial needs due to low patient census please reach out to the AM to discuss floating the contractor to a nearby clinic to pick up hours.
Types of Cases: Start of Care Wound Care Follow up Visits etc.
Years of experience REQ: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty
First-timers accepted: (Y/N) No must have recent specialty experience.
Weekend REQ: Varies per location and profession please have traveler discuss during the interview.
Certs REQ: CPR BLS

Territory/County covering: Territory mileage radius Must be willing to travel up to 75 miles from home clinic.
Mileage reimbursed IRS Standard rate. Traveler is compensated from patient home to patient home. The commute to and from the first and last patient are not billable.

Some positions specifically in the western territory often have a Baylor Shift schedule: 4x10s Friday-Monday. Most will be standard 5x8s M-F.

Nicole was very helping and always proficient with reaching out and even going above and beyond when the facility itself was slacking. When the facility canceled all contracts the day before start date she found me another position within the same week. I would recommend her 10x over.
