Description

Primary Care Clinic at Tsehootsooi Medical Center. Title : Care Coordinator (case manager) nurse positions within our Primary Care clinic: Requirement: Three (3) year Case Management Experience . Shift : Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays. Collaborate with support nursing staff and with providers for patient care. Housing: Information can be found under the 'Facility Group Document’ section. Orientation : on Mondays only - If your traveler misses the compliance deadline, they will be pushed to the following scheduled Orientation.

Nicole was very helping and always proficient with reaching out and even going above and beyond when the facility itself was slacking. When the facility canceled all contracts the day before start date she found me another position within the same week. I would recommend her 10x over.
