Position Description: Must have acute care adult hospital case management Unit Information: SOCIAL WORKER (917) Facility: HCA FLORIDA LAWNWOOD HOSPITAL Address: 1700 S 23rd Street, Fort Pierce, FL 34954 Floor: Building: Cost Center: 917 General Information Tell us about the unit(s): · Number of Beds: 344 beds at acute care hospital (excluding BH and acute rehab) · Patient Ratios: SW if pair with Matrix of another RN will work Complex case of 8 or 9. If they are just covering DC planning then will have 1:25 or up to 30 · Documentation System: MIDAS/Meditech, Nate, Navihealth, Care Team Assignment · Call Requirements: All work every third weekend · Floating policy: All Social workers float-except trauma. · Patient Experience requirements: Knowledge of Hospital DC planning/execution and knowledge of MDR or IDT Typical Hiring Profile Must have : · MIDAS/Meditech Experience, PASRR, Navihealth · MSW or LCSW List Typical Procedures Performed on Unit(s): · Discharge planning, MDR, Referrals, Working with Family and Patients Additional Preferences: (ex: previous travel experience preferred, easily adaptable to fast paced unit, can function well independently) · N/A Dress Code : · n/a Awards/Recognition: Feel free to include items such as links to videos that feature the department, outstanding department/team awards, etc. · n/a
