SUBMISSION PROFILE REQUIREMENTS - Must be in the profile or cannot move forward with submittal.
• Start Date – Must match the next available start date, include in attached profile/resume
• EPIC experience required. Ensure it is accurate (there have been inconsistencies on the resume versus the cover page)
• EMR listed on each work assignment
• Time Off- Ensure dates make sense for the required start date and are 5 days or less.
• Include within the attached profile/resume. All RTO needs to be in at time of submission.
• Resume- Ensure resume information is accurate (there’s been an increase in inaccurate information sent).
• MUST INCLUDE: detailed duties, hospital size, unit size, and trauma level (if applicable) for each hospital.
• Explain any employment gaps upon submission and include in attached profile/resume.
• Certifications- Make sure certs are accurate and up to date.
License- Active and in hand - AZ or Compact- Ensure it's accurate

- # treatments per day: 5-6 vents, treatments 35 in 12 hours.
- Certs: BLS, ACLS, PALS, if NICU exp-do train in NICU would need NRP also. Need RRT AZ. NO CRT!
- Types of patients: Adults, peds, neonates- only if NICU trained- will attend all deliveries and c-sections.
- What areas do they cover: Inpatient, Foothills ER- freestanding.15 min drive away- may float.
- Experience: 2 Years of experience is okay to have a first-time traveler.
- Job Duties: ABGs-draw and analyze, treatments, vent settings.
- Are RTs Required to be able to Intubate Independently or only to assist?: Travelers are not required to be able
to intubate but must assist.
- Hospitalist or Intensivist in house 24/7?: Intensivist in house 24/7.
- Is there a Respiratory Manager/Supervisor/Lead Therapist on each shift?: There is a Charge RRT on each shift.
- Would the traveler ever be the only RT in-house: For the main hospital will not be alone, and for Foothills ED would
be the only one there.
- RT Equipment: Vent-Avea, Hamilton C1, Bipap-V60, EV300, HiFlow- Aerovo 2. Zoll vents for backup. Glidescope
and Glidescope-GO for intubation.
- Shifts & Scheduling: 6a/6p. Don't guarantee a block schedule, but will try to accommodate if able.
- Weekend Requirements: Every other weekend.
- Holiday Requirements: Travelers will work 2 of 3 major holidays.
- Call or standby call requirement: Can be placed on "low census."
- Floating Requirements: May float to Foothills Freestanding ED-15 min drive.
- Scrub Color/Dress Code: Black Scrubs.
- How many shifts of unit/department-specific orientation: one NEO, one day ED, one day ICU, one day floor,
(would add additional day for NICU-if they have NICU experience).

Nicole was very helping and always proficient with reaching out and even going above and beyond when the facility itself was slacking. When the facility canceled all contracts the day before start date she found me another position within the same week. I would recommend her 10x over.
