Data Collection : Unit Information: Location: Building: Med Plaza 1 Floor: 4 th floor suite 411 Order Detail: Facility Department Number: 84624 Discipline: RN Nurse Case Manager, Licensed Social Worker Quantity: 1 of each Target Start Date: ASAP Shift: CM 8-430 SW 830-500 Call Requirements: none Weekends: rotating General Information Tell us about the unit(s): · Number of Beds -N/A · Number of Staff – 22 · Type of staff: o Day Shift – Case Managers, Social Workers, Case Management Assistants · Patient Ratios – 25:1 CM, SW 40:1 · Type of equipment – MIDAS/CAREPORT/INTERQUAL · EMR –MEDITECH · Permanent Positions on Unit Avaliable? (Y/N) –YES 3.5 FTEs Typical hiring profile Skill Set Most recruited for: (Travel RNs, local CNAs and sitters) Must have : candidates without these skills will not be considered for the role. · Case Manager: 2 yrs Acute Case Management experience · Social Worker: 1-2yrs Acute Social Worker experience (licensed in TX) Preferred or nice to have : candidates with these skills will be considered first. · See above · See Above and MSW List typical procedures performed on unit(s): · Case Manager: Provides Case management service for both inpatient and observation patients assigned. Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family, coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary team. Evaluates progression of care using InterQual throughout the episode of care. · Social Worker: Works collaboratively with RN case managers and the multidisciplinary team, performs assessments and develops appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs. Engages in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care. Ability to multitask and prioritize work flow and needs. Ability to adapt to changes, team player. Nicole was very helping and always proficient with reaching out and even going above and beyond when the facility itself was slacking. When the facility canceled all contracts the day before start date she found me another position within the same week. I would recommend her 10x over.
